Initial Phased Return Declaration Form for Nursery, Reception, Year 1 and Year 6

With reference to our letter dated 21st May 2020, could we ask that parents and carers with children in the above classes please complete and return this form using the submission button at the end of this page. This information will continue to assist us in our planning to re-open school to more pupils in line with Government and Local Authority guidance.

If you are already listed as a Key Worker or your child is classed as vulnerable and has been attending school throughout the lockdown, they will still be able to attend under current arrangements, regardless of which year group they are in.

Please respond by Thursday, 28th May. If we do not receive a response from you by this date we will make the assumption, for planning purposes, that you do not intend to send your child back to school at this time.
If, as explained within the initial letter that you will have received, you decide to send your child back to school at a later date you will need to contact the school office to discuss this. This is in order to allow us to allocate your child to a 'pod' and ensure that we do not go over the recommended maximum number of 15. You will then be advised of a start date for your child.
